Making a Birth Plan

A birth plan helps your midwife/LMC, your whanau and other support people know what your wishes are when it comes to giving birth.

Your midwife will sit down with you and discuss in detail the choices you should make ahead of time to create your formal birth plan. We’ve created this handy online questionnaire to help get you thinking about your birth plan.

Birth Plan Questionnaire Tips:

• You can find a lot of great tips for making your birth plan on the Kiwi Families website.
• Allow 30-60mins to complete the questionnaire. You can save your progress any time by clicking the Save Draft link.
• Skip any questions you’re not sure about, you can always discuss with your midwife/LMC later.
• At the end you will be able to email your completed questionnaire to yourself and/or your midwife.
